I'd reword Impossible Task a bit to something like:

When an enemy attacks an ally of the pledged, they may spend a use of resolve to move their speed as an immediate action toward the attacker, if this puts them adjacent to the enemy that triggered Impossible Task, they may make a single melee attack against them.


This allows Impossible Task's movement to trigger at any range, but doesn't give you the free swing unless you can close distance; it also specifies that the Samurai is actually moving since "relocate" is unclear on weather this is a regular movement or some kind of teleport.

Also might want to make the attack penalty a flat -5 or so (or perhaps a +5 dodge bonus for the ally), at 15th level it's not very hard for a martial character to be hitting for 20 or more damage on a single hit, which would negate any d20 attack roll (unless the monster had an attack bonus so high it was automatically hitting people without rolling anyway).

At that level, it's not uncommon at all for your enemies to have bonuses high enough to do just that.
